Geriatrician (SO) / Nurse Practitioner (NS) consultation
Are you a general practitioner and do you have a vulnerable elderly person living at home without a WLZ indication who has complex care needs? Do you require the expertise of a geriatric specialist or nurse practitioner? Request a consultation with us:
- Telephone: 020 – 445 97 55 (office hours)
- E-mail: eerstelijnbrentano@brentano.nl
Collaboration between GP and SO/VS
The General Practitioner is the first point of contact in primary care and is responsible for general medical care for older people living at home, in sheltered housing, or in an institution without WLZ treatment. Sometimes, the expertise of a Geriatrician (SO) or Nurse Practitioner (VS) is desirable.
Our geriatric specialists and nurse practitioners are engaged by the general practitioner for the diagnosis, advice, and treatment of physical complaints, psychological complaints, and/or behavioral issues. This can take place by telephone, during a consultation/home visit, or during a Structured Periodic Consultation (GPO) at the general practice. During a home visit, the geriatric specialist/nurse practitioner assesses the complaints, medication, and their effect on functioning and quality of life together with the patient and their relatives. Subsequently, the geriatric specialist/nurse practitioner advises the general practitioner. If necessary, there is consultation with other healthcare providers for coordination. Our specialists may temporarily become involved in co-treatment.
Compensation
The consultations with the SO are reimbursed by the health insurance of the health insurers contracted with us and are subject to the deductible.
We conduct these consultations exclusively with the general practitioners in the region to which we are assigned, and registration for this is done via email. Although this is usually arranged through the general practitioners, we can also conduct consultations for our practitioners in the neighborhood upon request.
