Our community nursing
Brentano District Nursing provides care and support to people in their homes seven days a week and 24 hours a day. We do this with five district teams. We are active in the municipality of Amstelveen in the postal code areas 1181 to 1183 and 1185 to 1188.
In addition to care and support, we also look at the possibilities, at what someone can do. The more control you keep over your life, the more vital and self-confident you feel.
Despite limitations, because there is a solution for much.
Our work is aimed at enabling people to live at home as independently and safely as possible. We make agreements with you and your family/caregiver regarding our care and support. We record this in a care plan. The district nurse works together with your GP, your pharmacy, and specialists. The district nurse can also refer you to a physiotherapist, a dietitian, or other support in your area. You will be assigned a contact person.

Case management
Should you or your loved one require help and support following a dementia diagnosis, you can engage a case manager. A dementia case manager Knows exactly how to navigate the jungle of regulations, laws, and possibilities in the field of care, and has experience with the mental health issues you may face. As an independent support worker, the case manager will keep you informed, provide emotional support, and arrange the necessary care for you.
