We support people with early-stage memory problems and (advanced) dementia who struggle to structure their day effectively on their own. Five days a week, we offer a pleasant and safe place with a clear daily routine. A meaningful day in a home-like atmosphere. Our day care ensures that people can often continue living independently at home for longer and that informal caregivers can take a breather.
We offer safety and, above all, a smile in a small-scale and homey atmosphere.
Our opening hours are Monday, Tuesday, Wednesday, Thursday, and Friday. Everyone is picked up and brought home. We arrange the taxi transport. On Monday, Wednesday, and Friday, a colleague from the day care center rides along to assist with escorting to and from the taxi.
The day care is located in nursing home Klaasje Zevenster. Address: Klaasje Zevensterstraat 103, 1183 NK Amstelveen.
For our Day Care services, an assessment decision is required from the Centre for Care Assessment (CIZ) or an assessment under the Social Support Act (WMO).
Case management
Should you or your loved one require help and support following a dementia diagnosis, you can engage a case manager. A dementia case manager knows their way through the jungle of regulations, laws, and possibilities in the field of care exactly, and has experience with the mental health issues you may face. As an independent support worker, the case manager will keep you informed, provide emotional support, and arrange the necessary care for you.
