Your support and refuge with dementia
More and more people with dementia are staying at home longer. That is wonderful, but also very demanding. How do you ensure it remains pleasant? For the client and their loved ones. Our dementia case management supports the client and their loved ones.
What does a case manager do?
A dementia diagnosis changes your life. Whether you suffer from the disease yourself or a loved one is diagnosed, a lot will come your way and you will eventually have to arrange things. A dementia case manager supports you in this and is, in many cases, an indispensable source of support and reassurance. Not only because this person knows their way around the jungle of regulations, laws, and possibilities in the field of care, but also because, through experience, they often know what you are dealing with mentally. As an independent support worker, the case manager will keep you informed, provide emotional guidance, and arrange the necessary care for you.
The central hub
Our case managers help with:
- Navigating the complex world of care and welfare;
- Applying for the Long-Term Care Act (WLZ) and finding a suitable place to live.
- Arranging care and support such as district nursing, day care, domestic help, meal service and home automation;
- Providing emotional guidance.
We work closely with Amstelring and Zonnehuisgroep Amstelland in this regard. The case managers from these organizations coordinate with each other and work with a joint waiting list. In this way, we ensure that clients receive appropriate support as soon as possible.
Registration takes place via the joint registration portal. During this process, it is determined where the support can best be taken up.
Compensation
Dementia case management can be requested via the general practitioner, falls under the district nursing entitlement, and is reimbursed from the basic insurance or the WLZ as long as the client lives at home.
