5-step model

5-step model

What can someone do themselves? What can the network do? What can technology do? What can other (paid) organizations do? What can the Brentano professional do?

Why do we care together?
Sometimes, support needs can be resolved in ways other than through the use of professional support. Therefore, we carefully assess what a client can do independently or what they can still learn. We also consider what the person's network can contribute. This allows us to organize care as effectively as possible, working together. This contributes to self-reliance, promotes personal autonomy, and ensures we remain connected to the surrounding environment. As a result, clients feel comfortable and secure.

Caring together with the 5-step model
We go through five steps to determine together what someone can still do independently, where the network, digital tools, or other agencies can provide support, and what professional care ultimately takes over.

What can the client still do independently?
Think about taking medication, keeping the room clean, watering plants, tidying the closet, and maintaining and cleaning the refrigerator.

Help from your loved ones and the network
When a client receives support through Brentano, the role of loved ones remains important. They can continue to help and support. Think, for example, of tidying up, cutting nails, combing hair, doing groceries (together), or organizing a fun outing; essentially any support you can imagine. We encourage this so that clients remain connected to their environment.

(Digital) tools
There are some things a person cannot do independently, but can with an aid. Think of a shower chair, video calling for remote contact, special cutlery for Parkinson's, or an automatic medication dispenser.

Other agencies
Sometimes we enlist the help of other agencies or individuals/volunteers. (For this, please refer to the list of external support services)

What does the Brentano professional do?
Our professionals handle what cannot be done otherwise. Naturally, we are happy to help, but unfortunately, that is not always possible. Therefore, we discuss together what you are still able and willing to do yourself, and what your network can do for you.

This is how we truly take care together
